A copy of your host's proof of residency is required with your application.

Tips for Completion:

• Look through your passport pages to find your entry and exit dates. 

• If you need more than ten entries, press the tab key in the last cell of row ten to create a new row. 

A signed and notarized consent letter is required from the non-traveling Parent (if only one parent is traveling with the child). 

You may download and complete the attached sample to use as your consent letter. Replace the blue areas with your own text, using the same color and font as the rest of the letter. 

Do not sign the letter until you are in front of the notary. 

Once you have completed and uploaded all of your application documents, you will need to sign your checklist and upload it as the final item for review. 

If the checklist is for a child's application one of the child's parents or guardians should sign in their place.
