Information for

BRAZIL VISA - ADDITIONAL REQUIREMENTS FOR MINORS

Minors under the age of 18 are required to provide additional documents for submission.

Documents to be signed by both parents/guardians and notarized:

  • Completed Visa application forms (2)

  • Official Letter of Consent with 2 passport photos of each parent (2) - see below

  • Travel of Authorization

  • Affidavit of Support - see below

  • Declaration of Non-Citizenship - see below

    Additionally:

  • A notarized photocopy (not the original) of the child's birth certificate is required for residents of most states.

  • Children 3 months - 6 years old: An International Polio Certificate of Vaccination is required.

    The documents above will be returned after the visa is processed and should be carried with the passport when traveling to and from Brazil.

    Children of Brazilian parents (either mother or father) must travel on a Brazilian passport.
